New Students and Kindergarten Student Registration

Please complete the registration documents. Visit the Forms and Resources section of our website for Registration Documents. Submit the completed forms and other required documents by one of the following methods.

Drop off at the school office
All students who are "new" to Dallas City Elementary must provide an original copy of the child's birth certificate. An original copy can be obtained from the county courthouse where the child was born. All students entering an Illinois school for the first time must also have a current physical on the proper Illinois form.
