Dallas City Elementary Looking for Volunteers
This is a reminder that if you have students enrolling in Pre-K, Kindergarten, 2nd grade or 6th grade in the fall they will need the following health exams:
New Pre-K Students
Physical exam and required immunizations including Pneumococcal, lead screen, diabetes screening by October 15th.
Certified copy of birth certificate by October 15th .
Physical exam and required immunizations including proof of 2 doses Varicella vaccine , proof of 2 doses of MMR vaccine, lead screening, diabetes screening by October 15th
Certified copy of birth certificate (if not on file) by October 15th
Vision exam by an optometrist or ophthalmologist by October 15th
2nd Grade Students
Proof of 2 doses Varicella vaccine
6th Grade Students
Physical exam and required immunizations including one dose of Tdap vaccine, proof of 3 doses Hep B, proof of 2 doses of Varicella vaccine, proof of 2 doses of MMR vaccine, MCV vaccine, and diabetes screening by October 15th.
Beginning this school year (Fall of 2014) ALL students entering, advancing, or transferring into grades K - 12th grades will be required to show proof of receipt of two doses of MMR vaccine and two doses of Varicella vaccine.
Failure to comply with the requirements of the health/immunization exam by October 15th will result in the student’s exclusion from school until the required health forms are presented to the school.
