Important Announcements

2014-2015 School Registration & Fees
Registration will be Monday, August 4, 2014 from10:00 a.m. until 6:00 p.m. and Tuesday, August 5, 2014 from 8:00 a.m. until 3:30 p.m. As we plan for the 2014-2015 school year it is very important for your child to be registered during this period. Please plan to register and pay fees on one of these dates.

Summer Library and Computer Lab

The Dallas City Elementary Library and computer lab will be open on Wednesday mornings from 9-12:00. Come in each week and get new books! Our computer lab will also be available at that time. Mrs. Ryan will be hosting our summer reading program.

This is a reminder that if you have students enrolling in Pre-K, Kindergarten, 2nd grade or 6th grade in the fall they will need the following health exams:

New Pre-K Students

Physical exam and required immunizations including Pneumococcal, lead screen, diabetes screening
by October 15th
Certified copy of birth certificate by October 15th
Kindergarten Students
Physical exam and required immunizations including Varicella 2 doses, proof of 2 doses of Rubella and Mumps vaccine, lead screening, diabetes screening by October 15th
Certified copy of birth certificate (if not on file) by October 15th
Vision exam by an optometrist or ophthalmologist by October 15th
Dental exam
2nd Grade Students
Proof of 2 doses of Rubella and Mumps vaccine
Dental exam
6th Grade Students
Physical exam and required immunizations including one dose of Tdap vaccine, proof of 3 doses Hep B, proof of 2 doses of Varicella vaccine, proof of 2 doses of Rubella and Mumps vaccine and diabetes screening by October 15th
Dental exam
Beginning next school year (Fall of 2014) ALL students entering, advancing, or transferring into grades K - 12TH grades will be required to show proof of receipt of two doses of Rubella and Mumps vaccine.
Failure to comply with the requirements of the health/immunization exam by October 15th will result in the student’s exclusion from school until the required health forms are presented to the school.
